Veteran Directed Care Program
A Self-Directed Home Care Support Program for Veterans
Veteran Directed Care (VDC) gives eligible Veterans more choice and control over the care and support they receive at home. Through a collaboration with Veterans Affairs, Disability Network Northern Michigan helps Veterans identify their care needs, develop an approved spending plan, and manage the services and supports that help them live independently.
Am I Eligible?
Veterans may be eligible for Veteran Directed Care if they:

Are enrolled with Veterans Affairs and eligible for community care

Meet the clinical criteria for the service
The VA Primary Care Team determines eligibility for VDC. If eligible, a referral will be made to the Veteran Directed Care program based on the Veteran's needs and interest.
How Disability Network Supports You
A Person-Centered Counselor from Disability Network Northern Michigan will work with you to identify your care needs and help develop a spending plan for the services and supports you need. Annual budgets and spending plans must be approved by the Veteran Directed Care Coordinator.
We Can Help You Plan For:

Personal care needs, including bathing, dressing, grooming, toileting, and medication management

Goods and services that support independent living

Respite and emergency care provided by others

Caregiver scheduling and support (caregiver can be a friend, spouse, relative, etc.)
You Direct Your Care
Veteran Directed Care puts the Veteran in control of many decisions about their care. Eligible Veterans are responsible for:

Hiring their own personal care aides

Purchasing approved items and services within their spending plan

Following program guidelines
-
Recruiting, interviewing, and training support workers

Supervising and evaluating support workers

Approving timesheets and determining employee wages
Disability Network Northern Michigan works with Veterans to help manage employer responsibilities and financial management service providers. No funds are paid directly to the Veteran.
